The information below should answer any questions you may have on entering a request for service or information into the system. If you need further assistance, please call Neighborhood Services at 757-7263.
These instructions will walk you through the complete request entry form. If you have a question about data for a particular field, each field is listed in bold below.
At the top of the form, check the list of most frequent requests to see if your request is listed. If listed, click on the bullet next to the issue that fits. If you do not see the concern listed, then click on “Other Requests”.
Request Field: Note: this field is to be used only with “Other Request”. Enter here a brief summary of the nature of the request. For example: if the problem is not on the list of most frequent requests, enter your concern in the other request section.
Request Description: This is the place where most detail can be provided on the service or information request. This should include as much information as possible so the city can direct the problem to the proper department. Please be as specific as possible on the nature of the request.
Address: Next enter the street number and street address of the location you are reporting. Please put all directional information on the end of the street. For example: 1930 Germantown Rd. S. instead of 1930 S. Germantown Rd.
The next fields are your contact information. All fields marked with asterisk* are required fields and must be filled in. Please be aware that any information entered here is subject to the Freedom of Information Act.
First and Last Name: Note please make sure to enter these into the separate fields. If you wish to remain anonymous, you can enter “A” in the first name field and “Anonymous” in the last name field.
Street Number and Street Name: Please make sure to enter these into the separate fields.
City and State: These fields are already filled in if you are a Germantown resident, otherwise type over the entered information and it will update.
Zip Code: Enter zip code.
Phone: Enter phone number in the format displayed.
Note: This number is used for tracking and log in purposes. Please make sure it is entered properly or you will not be able to log back into the system to check on the status of your concern. This field is case and character sensitive so it must ne duplicated exactly for future login.
E-Mail: If an email address is entered you will receive updates on the progress of your concern.
Submit: Once you submit your request you will be given an ID number. This ID number along with your home phone number will allow you to log back in to check the status of your request.
